Then after retrieving the file from your personal download directory, you will be able to add your personal details to it online, save the document to your files and when ready, send the completed form attached to an email to ESSC as per the details on the JQ.   Please note: the Word version or the PDF of the JQ can also be printed off and completed by hand.
